All patients should receive considerate and respectful care.
Patient should receive complete information concerning diagnosis, treatment, and prognosis from their physician in terms they can be reasonably expected to understand. When it is not medically advisable to give such information to the patient, the information should be made available to an appropriate person in his or her behalf. All patients should know by name the physician responsible for coordinating his or her care.
Patients should receive from their physician information necessary to give informed consent prior to start of any operative or high-risk procedure. Except in emergencies, such information for informed consent should include, but not necessarily be limited to the specific procedure and/or treatment, the medically significant risks involved, and the probable duration of incapacitation. Where medically significant alternatives for care or treatment exist, or when the patient request information concerning the medical alternatives, the patient should receive such information. Patients should also know the name of the person responsible for these procedures and/or treatment. Each patient has the right to be involved in the plan of care and treatment. They have the right to be informed of his/her health status and prognosis.
Patients may refuse treatment under most circumstances and should be informed of the medical consequences of such action.
Patients should receive every consideration of their privacy. Case discussion, consultation, examination, and treatment are confidential and should be conducted discreetly. Those not directly involved in the treatment should have the permission of the patient to be present.
Patients should expect that all communications and clinical records pertaining to their care would be treated as confidential.
Within its capacity, the hospital will make a reasonable response to the request of the patient for services.
When medically permissible, a patient being transferred to another facility should receive complete information and explanation from his or her physician concerning the needs for and alternatives to such a transfer.
Patients should expect that complaints or concerns about their care will receive prompt attention by appropriate hospital and/or medical staff officials. To voice a concern, contact the Administrative Supervisor at 267-3200 or ask anyone at the hospital to contact them. If concerns are not addressed to the patient's satisfaction, they may also contact any administrative staff member, Chief Executive Office, Chief Operations Officer/Chief Nursing Officer, Chief Financial Officer, or any Division Director or Director). Patients may also submit written complaints to Kosciusko Community Hospital, 2101 E. DuBois Drive, Warsaw, IN 46580, Attn: COO. Patients have the right to expect that the hospital and his or her attending physician will work cooperatively to assure reasonable continuity of care.
Patients have the right to examine and receive an explanation of the billing for services rendered by the hospital.
Patients should be informed of the hospital rules and regulations which apply to their conduct while hospitalized.
Patients have the right to be free from all forms of abuse or harassment.
The patient has the right to have a family member or representative of his or her choice and his or her own physician notified promptly of his or her admission to the hospital.
PATIENT RESPONSIBILITIES
Provision of Information A patient has the responsibility to provide, to the best of his knowledge, accurate and complete information about present complaints, past illnesses, hospitalizations, medications, and other matters relating to his health. He has the responsibility to report unexpected changes in his condition to the responsible practitioner. A patient is responsible for reporting whether he clearly comprehends a contemplated course of action and what is expected of him.
Compliance with Instructions A patient is responsible for following the treatment plan recommended by the practitioner primarily responsible for his care. This may include following the instructions of nurses and allied health personnel as they carry out the coordinated plan of care, implement the responsible practitioner's orders, and enforce the applicable hospital rules and regulations. The patient is responsible for keeping appointments and, when he is unable to do so for any reason, for notifying the responsible practitioner or the hospital.
Refusal of Treatment The patient is responsible for his actions if he refuses treatment or does not follow the practitioner's instructions.
Hospital Charges The patient is responsible for assuring that the financial obligations of his health care are fulfilled as promptly as possible.
Hospital Rules and Regulations The patient is responsible for following hospital rules and regulations affecting patient care and conduct.
Respect and Consideration The patient is responsible for being considerate of the rights of other patients and hospital personnel and for assisting in the control of noise, smoking, and the number of visitors. The patient is responsible for being respectful of the property of other persons and of the hospital.
